Risk factors: Adolescents and young adults (peak incidence); transmission via saliva.

When to seek evaluation: Severe fatigue, widespread swollen nodes, and sore throat together warrant a monospot test. Avoid contact sports while the spleen is enlarged due to risk of rupture. Call 911 or go to the emergency room immediately for sudden, severe abdominal pain (especially upper-left abdomen) during or after a mono diagnosis, this can signal a ruptured spleen.

3. Skin and Soft Tissue Infections

What it is: Infections in the skin (cellulitis, abscesses) or deeper tissues drain to the nearest regional lymph nodes, causing swelling in that area.

Why it happens: Bacteria (most commonly Staphylococcus aureus or Streptococcal species) enter through a break in the skin and trigger an immune response; the lymph nodes draining that area enlarge as they filter the bacteria and recruit immune cells.

4. Lymphoma

5. HIV Infection

What it is: Persistent generalized lymphadenopathy (enlarged lymph nodes in two or more locations outside the groin) is a common feature of untreated HIV infection.

Why it happens: HIV continuously stimulates the immune system, leading to persistent lymph node activation. Opportunistic infections in later stages can also cause localized lymph node swelling.

Common symptoms: Widespread lymph node enlargement; may be accompanied by fatigue, fever, night sweats, weight loss, and oral thrush; during acute HIV infection (seroconversion), swollen nodes with fever and rash may occur 2-4 weeks after exposure.

Risk factors: Unprotected sexual contact, IV drug use, needlestick injuries, vertical transmission (mother to child).

When to seek evaluation: Anyone with persistent lymph node swelling and risk factors for HIV should be tested. HIV is manageable with antiretroviral therapy, and early diagnosis dramatically improves outcomes.

6. Autoimmune Diseases (Lupus, Rheumatoid Arthritis)

What it is: Autoimmune conditions cause chronic immune system activation that can lead to lymph node enlargement throughout the body.

Why it happens: Persistent immune activation and inflammatory cytokine release in conditions like systemic lupus erythematosus (SLE) and rheumatoid arthritis stimulate lymph nodes as part of the ongoing systemic immune response.

Emergency, call 911 or go to the emergency room immediately if swollen lymph nodes come with:

  • Difficulty breathing or difficulty swallowing
  • In a severe neck infection: drooling, high fever, or rapidly spreading redness/swelling

These can signal a dangerous infection or airway/vessel compression that needs urgent treatment, do not wait for a routine appointment.

See a doctor soon (non-emergency, but should not be ignored) for nodes that:

  • Are larger than about 1-2 cm in diameter
  • Have been present more than 4 weeks without a clear infection
  • Are painless and hard or rubbery
  • Are growing
  • Are accompanied by B symptoms (night sweats, fever, weight loss)
  • Are in the neck of a person with smoking history (increased cancer risk)
  • Are located just above the collarbone (supraclavicular), these should be evaluated promptly regardless of size, as this location is more often linked to an underlying chest or abdominal condition

Any swollen lymph node in a child that is growing rapidly should also be evaluated promptly.
